To access the Emergency Medical Records for a LifeTrack member, please enter the ten digit LifeTrack number printed on the LifeTrack card into the User Name field.  For the PIN, enter the two digit month (for example 03 for March) and the two digit year of the birthdate (for March 20th, 1969 the PIN would be 0369).  Then click the "Log In" button.  If you have problems at any time please call 502-410-3774.  This information is to only be accessed by emergency medical personnel or healthcare providers who have been provided with permission to access the information.
